Did you set up all the buttons? I've forgotten to configure the controls of the game to the buttons on the controller plenty of times.
Are you plugging your controller in before or after you run the game? You have to have it plugged it before you run it.

Those are the only things I can think of that would make my controller sometimes not work in the game.
